Who we are?We are a young parents’ project, offering education, support and training opportunities to young mothers and mothers to be, aged 16–20. Our groups offer the opportunity to further your education, develop self-esteem and parenting skills, whilst empowering young people to make positive life choices for themselves and their children.Our OFSTED registered nursery has been rated as Outstanding. We employ qualified nursery day care practitioners, who provide a high quality, caring environment that promotes appropriate play and learning opportunities for children of young parents accessing B2b+ Where are we based?We are a city wide provision based in the Ryhope Health Centre. Bus fares to and from the venue are reimbursed where appropriate. Who can refer?We accept referrals from a wide-range of professionals – Social Workers, Early Help, Schools and Colleges, Family Nurse Practitioners, Health Visitors &amp; Midwifes, GP’s, Housing, Connexions, Housing and Support Workers. Young parents can also self-refer. Benefits of attendingB2b+ enables young parents to make new friends and reduces social isolation, whilst developing personal skills. Our courses include Health &amp; Social Care, English and Maths, Incredible Babies/Toddlers; our Wellbeing group is facilitated by Young Person Washington MIND Counsellor.Personal development sessions include independent living skills, parenting, cookery, craft, safety awareness, family away days, messy play and swimming.

This provider will welcome all children and will work with parents and professionals to try to meet individual children’s needs.

B2b+ Young Parents

The Community Child and Adolescent Mental Health Service (CAMHS) is a team of professionals who offer assessment and treatment to children and young people around their emotional well being. Our service works with children and young people aged 0-18 years old that may be showing signs of behavioural difficulties and / or emotional distress. Difficulties include worries / anxieties, low mood, controlling anger, bereavement and lose, bullying, eating problems, self harm and relationship difficulties. The service consists of a multidisciplinary team of mental health practitioners including, nurses, social workers, occupational therapists, health visitors, teachers, early years workers, counsellor and admin staff, all with additional child and adolescent mental health training. This includes Cognitive Behavioural Therapy (CBT), Systemic Family Therapy and Person Centred Counselling. The Community CAMHS service aims to deal with such difficulties in the early stages and focuses on prevention and early intervention.  The aim is to develop skills and resilience to promote positive well being / mental health and minimise the risk of further difficulties or distress. Following the acceptance of the referral, children and young people and their families will receive an assessment from one of our clinicians.  This will allow us to make the right decisions about what happens next.  This could be individual work, group work or in some cases a transfer to Specialist Mental Health Services.  The service aim is to ensure that children and young people are seen by the right service to meet their need. The service works within the NICE (National Institute for Clinical Excellence) guidelines around treatment interventions offered. The service has 3 locality bases across Sunderland, Washington and the Coalfield’s.  Families will be seen at the closest venue to where they live.  The service will also see children and young people and their families at other locations such as GP surgeries and schools. The service can provide consultation advice and support directly to families but also to other agencies working with children and young people presenting with mental health difficulties and will support the team around the child. Criteria for accessCommunity CAMHS is commissioned to provide a service of prevention of mental health problems, and early intervention when mental health problems occur, for children and young people aged 0-18. Aspects of clinical referrals which make them appropriate for Community CAMHS are: Mild to moderate mental health problems A brief history of presenting mental illness Lack of clearly established mental illness, but likelihood of developing one A strong community emphasis is likely to be necessary in resolving the problems An opportunity for preventative work is present There is some children and young people who may be presenting with the above but as their family circumstances maybe different they maybe more suitable for the specialist CAMHS. Health professionals, children services (including schools) and voluntary agencies referrals are accepted by letter or by strengthening families’ form which come via children’s services. The service operates from three venues across the city:<ul><li>    Valley Road Community Primary School (Main Office)</li><li>    Southwick Community Primary School</li><li>    Wessington Primary School</li></ul>Targeted servicesThe service is a targeted mental health service for children and young people and families with moderately severe mental health needs that cannot be met by universal or early intervention services. This includes the provision of a range of short evidence based mental health interventions delivered by CAMHS practitioners and counsellors.

Community Child and Adolescent Mental Health Service (CAMHS) Sunderland

Returners is an attendance provision for secondary students unable to attend mainstream education due to poor mental health normally associated with anxiety disorders. The aim of the provision is to provide a small nurturing environment where pupils can successfully access a broad curriculum, rebuild their confidence and self esteem, with a view to a successful return to mainstream education, a specialist provision or Post 16 education. They are based in Thornhill Academy where all pupils are taught and supported by a wonderful and dedicated team of professionals who not only see pupils for who they are now but for who they can become. An application for a place can only be made by the pupil's registered school via the Central Provision Panel.  The following criteria must be met:<ul><li>The young person's attendance must be less than 50%</li><li>They must be engaged with a Mental Health Service eg. CYPS or CAMHS</li><li>The young person does not display poor behaviour</li></ul>Please feel free to contact the Provision Manager Lyn White to arrange a visit.

Returners - Thornhill Academy

Providing awareness and support for adults who have dyslexia and assisting adults where this is causing barriers in their daily life, such as finding work, liaising with professionals, engaging with their children and other family members. Criteria for accessLive in Sunderland area and experience barriers because of dyslexia. Diagnosis not essential. Universal servicesLiaising on behalf of or supporting a person with contacting other agencies to seek any additional support needed. Signposting to other appropriate agencies with whom we are partners. Targeted services 1-1 Tailored support to adults who experience dyslexia, for example:<ul><li>Dyslexia awareness</li><li>Literacy skills</li><li>Confidence building</li><li>Self esteem</li><li>Coping strategies</li><li>Support in preparing for work, job searching or further training</li><li>Liaising with professionals</li></ul>Specialist servicesOffering free dyslexia awareness training and advice to professionals, volunteer staff, local agencies and other organisations, who work with people who may have dyslexia.

Special Olympics Sunderland supply coaching and training for youngsters with learning disabilities in swimming, judo, badminton and other sports.  We have qualified coaches in all sports.  Children and young people are encouraged to reach their full potential and enter competitions if they wish.  1-1 swimming lessons are available for age 6 years and over.Criteria for access All potential members must have a diagnosed learning disability and be aged 6 - 25 years.  All members are to be accompanied by a parent or carer.Targeted services Coaching and training for children and young people with a diagnosed learning disability.Cost to access services Fees of £4.00 per session and £20 annual membership.Please visit our website for more details.

Special Olympics Sunderland

The Wheelchair Service is based within the Occupational Therapy and Wheelchair Service.  It sits in the Personalisation Service within the People Directorate of Sunderland City Council. The Wheelchair service is made up of a Team Manager and 4 Wheelchair Assessors.  What do the Wheelchair Service do:<ul><li>Assess people who have presenting permanent mobility difficulty whatever their age or disability.</li><li>Provide manual and powered wheelchairs to meet their identified need, free of charge.</li><li>Assess for any additional postural or pressure relief requirements into the wheelchair.</li><li>provide cushions, accessories and special seating systems to meet their identified need, free of charge.</li><li>Work closely with the Regional Rehabilitation Engineering Service to address complex mobility needs</li><li>Operate a Wheelchair voucher scheme to facilitate Customer "choice".</li></ul>Criteria for access<ul><li>People who are registered with a Sunderland GP</li><li>People who demonstrate evidence of a permanent and diagnosed medical condition or physical disability that gives rise to mobility difficulties which will be experiences for longer than a six month period</li></ul>Targeted ServicesServices are offered to all age groups and all disabilities that give rise to a mobility issue. Sunderland Eye Infirmary Ophthalmology

YDAP offer help, advice and support on alcohol and drugs for young people, parents and professionals.  Criteria for accessYoung people accessing this service must be under 18 years of age, or if care experienced under 25 years of age. Young people accessing this service must live in Sunderland area.  In order for a referral to be accepted, the person making the referral must have consent from the young person or parent.  Targeted services Targeted brief intervention (one to one or group work) directed at known experimental or recreational users of alcohol and or drugs and also those young people deemed to be at potentially heightened risk and vulnerability. Specialist services Longer term individual service for more complex and serious cases.
